Health Services Forms
All medication forms, action plans, and health care plans are valid for one school year only and must be submitted at the start of each new school year, even if there are no changes to your child’s medical condition.
All health forms must be submitted directly to the School Nurse.
When a health form is uploaded to Genesis, parents/guardians are asked to contact the School Nurse to confirm that the form has been received and reviewed.
Parents/guardians are responsible for keeping emergency contact information current in Genesis at all times.
Timely submission of required forms helps ensure that medications, treatments, and emergency care can be provided without delay.

Concussion
Concussion Protocol/Student-Athlete (WTHS only) - Required to complete the RTP protocol with the Athletic Trainers before returning to PE/Sports Participation, which is provided to the student/family by the Athletics Department.
Concussion Protocol/Non-Student-Athlete - Student's physician submits a medical letter to the school nurse and status is monitored.

Middle School Students Only -
Athletics & Intramural Sports
Sports Forms are submitted to the Middle School Nurse - Adhere to all deadlines for submission of sports physicals. Student-athletes may not participate in interscholastic or intramural athletics, including practices or tryouts, until all required medical documentation is submitted and approved by the district physician and the school nurse. Approval may take up to 10 days, and clearance is not guaranteed for late submissions.
High School Students Only -
