MARCHING BAND FORMS 26-27
The following forms are required for all BHS Band members each school year.
NEW FOR 26-27- All forms are combined into one longer form.
Physical will be uploaded inside this google form. NO paper or email physicals will be accepted.
Fill out for all instruments, school owned & personal.
DUE May 1st uploaded into registration form above, no paper copies/emails accepted.
Medical history side parents fill out. The other side the doctor will complete during your physical appointment.
Students cannot participate in June Camp or any marching band activities without physical.

Physicals/Medical History form MUST BE COMPLETED BY MAY 1 and submitted on the Google Registration Form.
There is only 1 band form to fill out to register with the BHS Band, it is listed above, it is also where you submit the physical/medical history. NO PAPER COPIES OR EMAILS WILL BE ACCEPTED FOR PHYSICALS THIS YEAR, it is now part of your band google registration form.
Band Registration- April 11th- 10am-2pm
Mandatory June Camp: June 1st-4th. See calendar. (Percussion and Guard- 7am-4pm) (Winds- 7am- noon)
