The following forms are required for all BHS Band members each school year. Click the name of the form to fill it out via google forms. NO BAND FORMS NEED TO BE PRINTED, they are google forms.
Band Fees or Color Guard Fees (Google Form- do not print) Band do the "band fees" page and color guard do the "color guard fees" page. Filling out the band or guard fee form does not automatically charge your account and it is not you actually making the payment in Revtrak.

Instrument Check Out Form (Google Form- do not print) This is to be filled out by all students, even if they have a personal instrument.
PHYSICAL- THIS IS THE ONLY FORM YOU NEED TO PRINT...it is 2 pages. This must be turned in to Ms. Ballaro at BHS by May 19th. Paper copies only accepted. Students cannot participate in June Camp or any marching band activities without this physical.
