- Click on the add-on menu.
- Click get add-ons.
- Find Doc to Form.
- Allow Doc to Form.

- Click add-ons then Doc to Form.
- In the toolbar to the right, select whether you want the form to gather usernames, etc.
- Select the text for a question.
- Choose the type of question you want to create.
- Select the text for the answers, and separate each option with a semi-colon.
- Click view form to see what you have created.
