Once you've created the form, Email yourself a link to the View Form URL
From your mobile device, click on the link in the email
This will open in your device's browser
When the page loads your form, next open the menu window by clicking on the 'three dots' in the upper right
Scroll down the menu list and select Add to Home Screen
From there you can give it a descriptive name and you're done
Apple iOS devices
Steps for Apple devices is very much the same, only difference is instead of the settings menu you will see a 'Share' icon at the bottom of the browser window. Once selected you should be able to scroll to find the Add to Home Screen button.
