~Students who would like to participate in INTRAMURAL SPORTS activities (Open Gym) MUST also have a physical on file in order to participate~
If a student’s Pre-Participation Physical Examination was completed more than 90 days prior to the first day of official practice in an athletic season, a health history update questionnaire (see in "Important Documents") is also required to be turned in
FYI: Per the NJDOE guidelines for Sports Participation: "Students whose physical exam is up to date on the first day of the season (tryouts) are cleared for the entire season regardless of whether the physical exam form expires mid-season"
