***The mandatory Sports Physical form & Sports Sign Off forms MUST be completed and turned into the school nurse to ensure participation (SEE BELOW) ****
~Students who would like to participate in INTRAMURAL SPORTS activities at BME MUST also have a physical complete at least 1 week prior to participating~
If a student’s Pre-Participation Physical Examination was completed more than 90 days prior to the first day of official practice in an athletic season, a health history update questionnaire (see in "Important Documents") is also required to be turned in
FYI: Per the NJDOE guidelines for Sports Participation: "Students whose physical exam is up to date on the first day of the season (tryouts) are cleared for the entire season regardless of whether the physical exam form expires mid-season"
