Physicals

Every child who enters Pre-K, Kindergarten, or New Students entering a school district for the FIRST TIME MUST have a physical examination submitted.

• See "Important Documents" tab & submit "Universal Health Form" to school nurse via email or addressed in an envelope "For the nurse" and dropped off at main office.

All children who want to participate in intramural and interscholastic sports MUST have a valid sports physical on file.
