Physicals

Every child who enters Pre-K, Kindergarten, Fifth Grade, and New Students entering a school for the FIRST TIME MUST have a physical examination

• See "Important Documents" tab & submit "Universal Health Form" to school nurse via email or addressed in an envelope "For the nurse" and dropped off at main office

All children who want to participate in intramural and interscholastic sports MUST have a valid physical on file
