Prior to starting therapy at Willow Pediatric Therapy, we complete an evaluation to determine where we can help your child. Based on your concerns, we will help determine which of our evaluation packages is the best fit for your family and needs.
While non-traditional, we offer only packages for our evaluations which all include an initial home program and meeting with the caregiver to review the evaluation results and write up. We find this gives a strong start to the therapy process and provides families the knowledge they need to feel confident in the plan for their child's occupational therapy.
This evaluation would only be appropriate when there is only one area of concern such as handwriting.
This evaluation includes:
Simple record review of intake forms
45min-1hr of evaluation primarily focused on parent interview and clinical observations
Brief evaluation write up identifying strengths and areas of need
15 minute zoom meeting to go over the results of the evaluation
This is our standard and most common evaluation. Ideal for kids with challenges across settings including difficulties with feeding, attention, sensory processing, coordination or executive functioning difficulties.
This evaluation includes:
Record review including intake forms and medical records
1-1.5 hours of evaluation including standardized testing, parent interview, sensory processing review and clinical observations
Briefly reviews multiple areas of development such as sensory processing, gross motor, fine motor, coordination, self-care, executive functioning, social skills, and/or behavior
Standard evaluation write up identifying strengths and areas of need
Initial home program
30 minute zoom meeting to go over the results and initial home program
This evaluation is appropriate if there are concerns in a few areas including those children with confirmed or suspected diagnoses of Autism, Cerebral Palsy, Down's Syndrome and/or global developmental delay.
This evaluation includes:
Extensive record review including intake forms and medical records
1.5-2 hours of evaluation including standardized testing, parent interview, sensory processing review and clinical observations
Covers multiple areas of development such as sensory processing, gross motor, fine motor, coordination, self-care, executive functioning, social skills, and/or behavior
In-depth evaluation write up identifying strengths and areas of need
Initial home program
45 minute zoom or in person meeting to go over the results including caregiver education, review of the initial home program and time to answer caregiver questions.
During the evaluation, we may do a variety of assessments (could be standardized or simply observation). We may ask follow up questions from your intake questionnaires about your child's development and your concerns. If you have selected a "standard" or "comprehensive" evaluation, some of the assessment may not appear to be related to your primary concerns but doing a complete evaluation helps us find patterns in your child's challenges and identifying difficulties in underlying skills (for example, poor postural control can affect handwriting).
Specific skills that may be assessed during the evaluation include:
Sensory processing skills and abilities including strengths and differences
Fine motor skills (small muscle movements of the hand used for activities such as fastening buttons)
Gross motor skills (big muscle movements including walking, stairs etc)
Motor planning and coordination
Postural stability including core strength and shoulder girdle stability
Self-care skills such as helping with clean-up, dressing themselves, potty training, sleep
Feeding skills including oral motor skills, sensory sensitivities, cup drinking, utensil use
Visual perceptual skills, visual motor integration, hand-eye coordination
Executive functioning skills for older children such as attention, sequencing, self-control, planning ahead, flexible thinking, problem solving
