Applications are available on the link or in the Counseling Office.
Please fill out the fillable application. You must have a proof of physical on file with East High School within the past year or you need to provide an updated physical. Please review the attached and know your rights. NYS Child Labor Law Card for Workers (P886), and NYS Child Labor Law Card for Employers (P887)
Please sign and return the application to dcbauer@williamsvillek12.org
Once processed students can pick up their card in the Guidance Office. If it needs to be mailed, please provide an address and allow 3-5 days business days.
