GETTING YOUR WORKING PAPERS

If you are going to work part-time or during vacations:

1. Complete Part 1 -Parental Consent of this Application for Employment Certification including a Parent or Legal Guardian signature.

2. Provide only 1 of the following proofs for the Physical Fitness Certification:

a.Signature from the School Nurse certifying that you have had a physical within the last 12 months.

b. A copy of your physical indicating that you are physically fit for work and/or sports within the last 12 months.

C. A completed and signed Physical Fitness Certification from your Doctor's office. Note: the certification MUST have a physician's signature and doctor's office stamp.

3. Bring the completed documents listed above to the Guidance Office. APPLICANT MUST APPEAR IN PERSON

4. We can use your information in WITS for verification of birth date. If you are not a Williamsville North student, please provide a copy of your birth certificate, passport or driver's/learner's permit.

• • If you have dropped out of school and are going to work full time-your parent MUST accompany you to the Guidance Office for signature verification. Also provide the above information.

• • If you have graduated and going to work full time, you must bring a copy of your diploma to verify graduation. Also provide the above information.

• •  If you have a physical disability/limitation that limits your employment, you MUST have your employer fill out and sign PARTIV of the Application for Employment Certificate. Also provide the above information. This card is valid for only 6 months.
