Wrestler Forms and Physical Cards

Description of Forms (links below lead to PDF files):

Physical Card - To be signed by a doctor after a physical was done. Physicals are only needed every other year.

Alternate Year Card - To be signed by a parent the year after the last physical was done.

Athletic Code Agreement- To be read and signed by parent and wrestler every year. We want our athletes to be of the best moral and ethical character.

WIAA Medical Release Form - If you recovered from an injury, or skin condition please have the doctor fill out EVERY portion of the sheet. If any part of the sheet is not filled out you will not be able to wrestle until it is.
