Send/share/post the link to the assessment for your students.
Email Susan Bergman when your entire class has submitted the assessment.
As a reminder, these assessments are based on the previous grade level curriculum (so it should be content the students are already familiar with).
Directions for Students:
You are going to complete some problems so I can see what you already know and can plan our learning for the year. You will need something to write on and something to write with. The problems are in a google form and there are about 20 problems total. The first question will show you a list of teacher names, make sure you choose my last name. In the second question you will enter your first and last name. When you finish all the questions make sure you press submit and you see a screen that says "Your response has been recorded."
*5th grade teachers may want to add, "If your answer is a fraction, use one of the slash buttons (on the same key as ? or the key above the enter button) between the numerator and demoninator." (If they don't use one of these buttons, but in any way indicate their answer is a fraction, it will be considered a correct answer).
