Note: National Honor Society requires 30 hours of volunteering per year.

Step 1: Print and fill out both pages in it's entirety. Fill out all pieces and make sure you have signatures. (Remember that your parent and employer need to sign the papers.)

Step 2: Please bring printed and signed copies to the front office. The front office will ensure that they are accurate and if so, will take your working papers.
