WWPS offers free meals (breakfast and lunch) to all students at no cost to families through a program called Community Eligibility Provision.

The "OVER THE COUNTER MEDICATION" form is to be filled out, signed, and returned on a YEARLY BASIS.
This Medication Authorization Form is for any other medication needed by your student in the health office and administered by the nurse. These may include daily medications not listed on the "OTC" order form and daily medications considered controlled. These medications must be dropped off in person to the nurse by an adult over the age of 18.
This Asthma Action Plan form is to be filled out and signed by a doctor and parent. This medication order is valid for one school year and must be returned with the medication in its original packaging. A student may "self carry" their own medications, however they still require a current doctor's order on file.

This Religious Immunization Exemption Certificate must be on file for any student not receiving required vaccines for religious reasons.
