We have two system-wide ways we get to know our students.
The human side- mentoring and building relationships
The data side- the student personalized learning plan (PLP) in our LMS called Slate.
Mentoring is the foundation of all we do. One adult and a group of students come together to form a mentor group. The mentor is the first person of contact for each learner and their outside support.
Mentoring is not a class during the day. It is a time set aside for learners and their mentors to come together to:

The PLP: Personalized learning Plan
The Personalized Learning Plan
The PLP is the one place all student data can be found and is collected for ease of communication and decision making. There are several dashboards, but here we see two major ones: The Progress Report and My Profile.
The Progress Report communicates like a report card. My Profile is almost social media like, but asks about student preferences for learning.
Both are a great example of how students are seen as individuals and how we can personalize their learning.
