Name/Gender Changes
If you need to change your name/gender, complete the following forms and scan them to HR@weehawken.k12.nj.us:
https://www.state.nj.us/treasury/pensions/documents/forms/fc0369.pdf (This form needs to be printed, signed in ink, notarized and returned to the Business or Supt Office. You should not scan/email this form.)
Send a copy of your updated driver's license, SS card or passport (with new name/gender.)

New Address/Change of Address
Please complete the following and send them to HR@weehawken.k12.nj.us
Non-resident Parking Permit (If you moved out of Weehawken)
Health Benefits Enrollment Guide (you may update your address by following this attachment)
Don't live in NJ anymore? You need a residency waiver to continue working here.
If you don't live in NJ or recently moved out of NJ, you need to complete this waiver. Please complete the form as best you can and send it to HR@weehawken.k12.nj.us asap with a short letter explaining why you had to move.
