This needs to be filled out in BLUE ink by a medical doctor. The doctor must see you and clear you to wrestle. If you need any medication you will need to make a doctor's appointment. Usually in 48 hours, any infections are cleared but you need to see the Dr. again for him to sign off on your form. Make sure the entire form in filled out including the date, date of treatment, and date you are cleared to wrestle.
