Transcript Requests
The School Report Portion of the College Application
Once you have completed your application and clicked submit, there is a portion of the application that your counselor is responsible for. The forms below are required in order to follow through on that process. Your school documents will not be sent without the use of these forms.
Initial Transcript Request
This form is due at least two weeks prior to the college application deadlines. It is preferred that you give more than 2 weeks. All transcript request forms should be submitted by December 1, 2020. You can complete the form by clicking on and opening the Google Form to the left or the link below.
First Quarter and Mid-Year Transcript Request
ONLY TO BE USED AFTER THE OTHER INITIAL REQUEST HAS BEEN COMPLETED.
Some schools require updated transcripts for 1st quarter and/or Mid-year grades. Please use this sheet to request the necessary transcripts. Again, please give at least 2 weeks notice.
