Families that are returning to In-Person learning starting April 8 will be asked to complete several different forms prior to returning back to school. Some forms can be completed online while others will need to be printed, signed, and returned to school via email or dropped off in-person. Electronic forms can be emailed to mcoreasposada@wcpss.net
Every student who will ride District-provided transportation in a WCPSS Yellow School Bus must complete a one-time Online Health Attestation Form.
This form only needs to be completed once to be eligible to ride WCPSS Transportation for the remainder of the school year. If a student has already completed this form, the student does not have to resubmit the form for the transition to Plan A.
Read the Student Accountability and Required Health Protocols. Then use the link above to complete the student accountability form. WCPSS has created a Student Accountability and Required Health Protocols Form requiring parents/guardians to sign and submit to our school.
By signing this form, students and families are certifying that they have read and understood the Student Accountability standards that WCPSS has created for In-Person instruction.
Every student returning to campus for Plan A In-Person Learning must complete this form and return it to school. Families can complete the signature page and email it to Ms. Coreas-Posada, our Student Services Receptionist, at mcoreasposada@wcpss.net. If you are not able to complete this form electronically, we will provide paper copies for students when they return for In-Person learning and this form can be completed at that time. If a student has already completed this form, the student does not have to complete it again.
