New Student ENROLLMENT Information

*All new student enrollment forms are completed online.  Carefully read the information below before completing the registration.

New Student Registration

Once you have completed the online registration process, you will be contacted by phone to set up an appointment for the building your child will attend*.  Students must be accompanied by a parent/guardian and have the following documentation with them:

• Birth Certificate

• Most recent grade card/transcript

• Address of previous school

• Custody papers, if applicable

• Most recent IEP if in special programs

• Proof of Wauseon residency

*Please note:  If you are completing your registration during summer break you will be contacted after August 1st.
