The following information is necessary and/or required in order for us to ensure the protection of all children. There may be other documents that your child's school will ask you to complete. We appreciate your cooperation in providing this information.

Proof of residence (i.e. utility bill, lease agreement, etc.) or notarized "proof of. residence" form (required if recently moved into WCPS attendance area
Birth certificate (if one is not already on file with WCPS)
Physical examination form completed by a physician*
Current immunization certificate (must be on KY form)
Copy of custody documents (if applicable)
Copy of Social Security Card (if one is not already on file with WCPS)
