Enrollment Documents

The mission of the Warren County Public Schools is to prepare our students to think independently, to learn throughout life, and to be happy, healthy, productive members of a democratic society.

We are looking forward to working with you and your child. The safety of your child and all other children enrolled in our district is our first priority. The following information is necessary and/or required in order for us to ensure the protection of all children. There may be other documents that your child's school will ask you to complete. We appreciate your cooperation in providing this information.

*These forms and documents are to be completed AFTER you have been accepted into Beacon Academy.

Warren County School District

• WCPS enrollment form. click HERE to print the document.

• Proof of residence (i.e. utility bill, lease agreement, etc.) or notarized "proof of. residence" form (required if recently moved into WCPS attendance area

• Birth certificate (if one is not already on file with WCPS)

• Physical examination form completed by a physician*

• Current immunization certificate (must be on KY form)

• Home language survey

• Vision examination*

• Dental screening*

• Copy of custody documents (if applicable)

• Copy of Social Security Card (if one is not already on file with WCPS)
