By state law, all athletes must complete and turn in these 3 forms. Follow the links at the bottom of each form to the next form. Additionally, at the end of the 3 forms, there will be a link to start the process for a different student, if needed.
Per league regulations, all athletes must complete and turn in this form. It must be signed by a medical professional. Please print this out and bring to the student's physical appointment. A medical form provided by your physician or nurse practitioner is also acceptable as long as it clearly states the student has been cleared for participation in athletics.
