The Required Forms Checklists are mandatory face sheets for any and all IEP related documents that you complete and send to the District Office for scanning.
You will need to complete the correct checklist for each document (Example: You would complete an Initial IEP Checklist for an Initial IEP).
If you are missing a required form or signature, please note this on the Checklist.
If you are in need of more forms, please reach out to Amanda Olguin and request to have an additional RFC pad sent your way.
Important Reminder: All documents must be sent to the District Office for Scanning within 10 days from when the meeting was held.

Within 46 calendar days from date that the signed Assessment Plan is received by school, all assessments and reports must be completed and the IEP meeting held.
Timelines stop and re-start again when off-track time and school vacation exceed 5 school days.

A great way to engage with the Rubric would be to select an IEP that you developed and ask that a Content Specialist (TOSA) or Program Manager review the same IEP using the Rubric and then meet to compare your scoring of the IEP.

A great way to engage with this checklist would be to request that your site administrator, Content Specialist (TOSA), or Program Manager meet with you prior to an IEP meeting, attend the IEP meeting, and meet following the IEP meeting to work through this document. Schedule a time to then meet and have a discussion around the areas that fell outside of the "adequate" column.
