Guidelines for the enrollment process will be sent to families after registration/fee payment for camp is confirmed.
STEP 1:
Enroll in 4-H Online HERE. This will provide us with your child's "signed" Health History form, Code of Conduct, and other pertinent information that we will need to prepare for 4-H Camp. Please make sure you pay attention to these details when you enter your child's information:
In the Clubs section, please choose 4-H Junior Summer Camp 2022
In the Projects section, please choose Personal Development
In the Health Form section, please fill out all fields including any dietary restrictions, last tetanus shot date, physician's name and contact information, your health insurance policy number, and any health conditions that your child has.
In the Consents section, please respond to the media release, privacy statement, and code of conduct questions.
If your 4-Her is already enrolled in 4-H Online for the 2021-2022 4-H year, Extension staff will add the 4-H Camp unit to your enrollment.
If you have any issues enrolling, this Tip Sheet may be helpful or contact your local Extension Office. This step should be completed no later than May 15.
STEP 2:
Event Medication form: If your child will need medication at camp, this form must be completed. You do not need to fill this out for over-the-counter medications like Tylenol or hydrocortisone cream unless it is something your child uses every day. The 4-H Center carries medications like this and will give them to campers as needed. The 4-H Event Medication form is available HERE. Family must then return the Medication form (one for each child in family) to the local Extension Office. This can be done via email, fax, scan, mail or dropped off at the local office in accordance with local office procedures.
Completed event medication form (if needed) is due to your local Extension Office no later than May 15.
