The opening cutscenes in 1985 play out in much the same way as they do in canon, with a few exceptions. Namely, Lily does not exist. Saejima and Majima talk about the upcoming hit on the Ueno Seiwa clan, which they’ve been told will eliminate every major player from the clan, removing the threat to the Tojo currently in place. Saejima prepares his guns.
Saejima arrives at the ramen shop but Majima doesn’t show up. Saejima instead goes in personally with all his guns, killing everyone inside. Actually killing. There are headshots. Saejima is imprisoned for 25 years before being transferred.
Saejima arrives at the new clandestine prison and is subjected to brutality from the prison guards, headed by Saito. A brutalized Saejima is approached by Goh Hamazaki. The two of them talk about their shared Tojo past and ultimately resolve to escape together. Hamazaki brings up Majima’s “betrayal” to motivate Saejima further.
Once they have everything they need, Saejima and Hamazaki begin their escape plot. Hamazaki takes a ledger from the warden’s office as leverage before they escape. The two take out Saito and decide to become brothers with one another. Hamazaki tells Saejima to find a man named Kiryu.
As they’re about to escape, Saito comes back and wrestles with Hamazaki. Hamazaki and Saito tumble into the ocean. Saejima dives down after them.

Saejima washes up on the shore and struggles for a while before crashing to the ground. Haruka finds him and takes him in. Saejima recovers and speaks with Kiryu, desperately wanting to return to Kamurocho. Kiryu uncovers the truth about him as an escaped convict.
The two ultimately fight one another to a standstill but Saejima’s injuries get the better of him. He recovers more.
Saejima ultimately leaves on good terms with Kiryu and Haruka, who give him clothes to wear as he heads back to Kamurocho.

Saejima arrives back in Kamurocho and is forced to adjust to how things are 25 years later. He tries to locate his former family but finds nothing of note. While searching, he runs into Kido, who is on the run as police are pursuing. The two of them fight off police together and manage to escape.
Saejima and Kido talk some, getting each other up to speed on things. Kido is worn-out and tired from trying to evade both the yakuza and the cops. Saejima decides to offer Kido his protection despite barely knowing him. The two of them search for a place to stay, and are approached by an agent of the Florist, who invites them into Purgatory.
Saejima and Kido arrive in Purgatory. The Florist offers Kido asylum, which Kido gratefully accepts. The Florist and Saejima speak about what’s going on. Saejima wants to know the whereabouts of his former boss and Majima, who has apparently also gone dark within the Tojo Clan. Daigo Dojima and Yoshitaka Mine are cracking down on the remaining yakuza families, keeping them tightly in line to not give the police anything more to work with. Majima refused to cooperate, and his entire family is now at odds with the rest of the Tojo.
That’s all the info the Florist is willing to give out for free. For more, Saejima must engage in a death match with his champion, kill or be killed. Saejima reluctantly accepts.
The fight goes on. Saejima wins. He refuses to kill the loser, instead shaming the crowd as he yells at them about the horrors of killing. He starts to cry. The Florist is pleased.
The Florist tells Saejima everything he wants to know, showing him his former boss as Saejima cries. Saejima is then informed that the Majima Family has set up camp at Kamurocho Hills itself. Saejima says farewell to Kido and leaves.

Shibata and a few other patriarchs speak to Daigo and Mine. They are complaining about the intense scrutiny they’re currently under from Daigo and his men, saying that it’s unnecessary. Daigo insists that he is only trying to make sure that another incident doesn’t happen again. The Ueno Seiwa are causing a ruckus, and have apparently been starting to attack some of the Tojo Clan’s property, but if the Tojo Clan strikes back the police are primed to arrest anyone involved.
Daigo and Mine speak alone. Daigo says that they’re going to have to handle Majima sooner or later, but isn’t sure how. Mine says that he’s received word that Taiga Saejima is back in town, and is out for revenge. Perhaps their problem will solve itself. Daigo is displeased, but can do nothing about it.
Saejima is accosted by cops led by Sugiuchi, all of whom have an arrest warrant out for them. Sugiuchi expresses disdain for Saejima for his crimes, saying that all he deserves is a long life in the slammer. Saejima calls it torture. Sugiuchi doesn’t disagree.
Saejima manages to shake off his pursuers and heads to Kamurocho Hills. He’s greeted by Minami, who makes it clear that Saejima’s going to have to fight his way through. Saejima agrees gladly and slams his way through the entire Majima Family.
Eventually, he makes it to one of the upper, not-finished floors. Majima himself emerges, serious as Saejima looks back at him. The two move away in a car and head to the batting cages as Majima avoids the truth of what happened. Ultimately, the two fight with one another, and Saejima wins.
Majima and Saejima clear the air, and Majima ultimately confesses the truth of what happened. Shibata had ordered him against joining Saejima, but Majima had tried to refuse. He’d been tortured and lost an eye as a result of that. Saejima grimly understands what happened for the first time, and asks what’s next.
