Reimbursement Form
To submit a receipt for reimbursement:
Fill out a Check Request Form. (You can print them out here from the link below or find them in the PTA box in the workroom at school. )

Date you submit the request
Who to make the check out to (usually your name)
Address of who the check is made out to (usually your address)
Phone Number of who the check is made out to (usually your phone number)
Purpose: the PTA program or event you purchased items for.
Your name for "Submitted by:"
Attach the receipt to the Check Request Form.
Make sure the receipt only has PTA reimbursable items
Submit the completed Check Request Form by putting it in the Check Request Form Envelope in the PTA Box in the workroom or turn it in to a member of the PTA board.
Our PTA is governed by a set of Bylaws. The Bylaws are active for 3 years, then need to be reviewed and changed as necessary, approved by the PTA membership, and submitted for renewal with Utah PTA. Our current Bylaws expire January 1st, 2023.
Standing Rules contain job descriptions and more detailed information about conducting the business and directing the activities of the PTA. They are reviewed and renewed yearly as needed.
All board members need to be familiar with the Standing Rules. All committee chairs need to be familiar with their section of the Standing Rules.
The nominating committee is an elected group of PTA members. They are tasked with finding the best person for each elected office. They research each candidate’s qualifications and get consent from each nominee.
