Physicals & Participation Forms

NEW this year ALL Forms required by the BSD & Westview Athletics Office will be submitted ONLINE.  This is the first step to registering for Boys Soccer CAMPS & TRYOUTS.

Westview Athletics - Athlete Registration 

• • To Register your athlete:

    1. Go to the BSD Online Payment system https://or-beaverton.intouchreceipting.com/.  

    2. Log in.

    3. Select your student

    4. Under SHOP, select Register for Athletics and Activities.  

    5. Go to Boys Soccer Registration and click the Register button.

    6. Fill out all the forms.

    7. If needed, upload a current Athletic Physical Form.

2. Iniciar sesión

3. Selecciona tu estudiante

4. Bajo “SHOP”, selecciona “Register for Athletics and Activities”

5. Ve al “Boys Soccer Registration​ 2025-2026​” y haz clic en “Register”

6. Llena todos los formularios

7. Si se necesita, sube un formulario físico atlético en Athletic Physical Form

Physicals

• New physicals are required for ALL incoming Freshman and incoming Juniors. 

• All others will want to verify that their physical is still valid with the Athletic Office.

• Physicals are valid for 2 years.

• Current Form: Athletic Physical Form

• Where to get a physical: Your family doctor or someone from the Sports Physical List 

Turn in your forms in ONLINE ASAP PRIOR to Summer Camps, Weight Room, and TRYOUTS.
