About The Program: The Student-Led Longitudinal Diabetes Intervention Program is a CHIP program where student volunteers can help uncontrolled diabetic patients (A1c >10, >3 months since last appointment) from Wesley improve their blood sugar levels. Volunteers will be paired with a patient for 3 months, calling once a week to assist with diabetes care and education. Students will help patients address barriers to blood sugar control through motivational interviewing and advocacy for care.
This program serves as a great opportunity to:
- Positively impact patients within the local Phoenix community by aiding in their control of diabetes
- Work with a patient’s physician to understand the care plan for blood sugar control and work to lower their Hemoglobin A1c after 3 months
- Help patients overcome any social determinants of health to obtaining care or medications
- Utilize motivational interviewing to set lifestyle and preventative health-related goals like focusing on exercise, nutrition, and mental health
