A valid state issued identification card or military ID (for the child)
Copy of Childs birth certificate
Sports physical completed within the calendar year. Must have physician signed AYF Medical Clearance Form or NCHSAA physical form

**SIBLING DISCOUNT: $25.00 OFF REGISTRATION FEE**
NO REFUNDS UNDER ANY CIRCUMSTANCE AFTER 8/1/2025. All refunds are subject to board approval as well as amount to be refunded. Once uniforms are ordered that money will be deducted in the case of a refund.
