• Must have physician signed AYF Medical Clearance Form or NCHSAA physical form

• Complete registration form

• Registration fee: $150

Football Requirements

• A valid state issued identification card or military ID (for the child)

• Copy of Childs birth certificate

• NCHSAA physical form

• Complete registration form

• Registration fee: returning: $150           new: $180
