Maureen Albertson, DNP, RN, CHC, HCS-O
DIRECTOR OF HOMECARE COMPLIANCE • MILLENNIUM HOMECAREMaureen Albertson, DNP, CHC, HCS-O, RN, is the Director of Homecare Compliance and Quality Assurance for Millennium Home Care LLC. Maureen also serves as the Millennium Home Care Incident Commander as part of the Emergency Management Team. In that capacity, she serves as a Coalition Member for the agency on the Southwest Florida Healthcare Coalition. Maureen serves as a committee member on the Partnership for Quality Measurement (PQM) Post-Acute Care-Long-Term Care Committee with Battelle, a Centers for Medicare & Medicaid Services (CMS) certified consensus-based entity.
In the home health sector, Maureen has served as a Nurse Case Manager, Quality Assurance, Supervisor of Clinical Services, Staff Development Coordinator, and Compliance Officer. She is the author of Decreasing Urinary Incontinence in Home Healthcare and an advocate for best practices in the home setting. Maureen holds a Doctor of Nursing Practice (DNP) from Capella University and Master of Science in Nursing Education from Chamberlain College of Nursing. Maureen is a member of the Health Care Compliance Association (HCCA) and holds a certification in Health Care Compliance (CHC) and Home Care Clinical Specialist – OASIS (HCS-O) certification in the Outcome and Assessment Information Set (OASIS). She previously served on the HCAF Board of Directors and is a member of the HCAF Education Task Force. She has been a registered nurse for 32 years.
Denise Bellville, RN, BS, COS-C
EXECUTIVE DIRECTOR • HOME CARE ASSOCIATION OF FLORIDADenise Bellville, RN, BS, COS-C, is HCAF’s Executive Director. She has been a registered nurse for 45 years, working in critical care, ER, and open heart surgical intensive care unit (SICU) and neonatal intensive care unit (NICU) before transitioning to the home care industry as an executive for 30 years. Her home care clinical experience ranges from pediatric/adult ventilator cases, infusion therapy, and in-home peripherally inserted central catheter (PICC) line placements. Her favorite aspect has always been teaching elderly patients information that helps them reach their potential and remain at home.
“I knew I wanted to be a nurse since I was five years old. The only thing I asked for that Christmas was a nurse’s bag, and once I found it on Christmas day, I told my parents to just have Santa take everything else back. My first introduction to home care was a pediatric ventilator case. The hospital asked if I could get her home, settled, and teach the family what they needed to know. Well, as you might suspect, I was there for two years. My love of nursing has allowed me the opportunity to enter a stranger’s home and leave as a friend. Even after all these years, those new friends still invite me to family affairs. It has been a career that has already lasted a lifetime.”
Charles Bibbins
SENIOR PROVIDER RELATIONS REPRESENTATIVE • PALMETTO GBAMichael Bibbins has more than eight years of experience working in the health insurance industry. He joined the Provider Outreach and Education team after spending three years in the Palmetto GBA Provider Contact Center. He holds a Bachelor of Science from the University of South Carolina and a Master of Arts in Strategic Communication from the University of Delaware. Michael currently serves as a Senior Provider Relations Representative for the Review Choice Demonstration (RCD). As part of his responsibilities, Michael conducts workshops, educational exhibits, and web-based training for providers concerning RCD coverage and billing guidelines.
Elizabeth Brogdon
SENIOR EDUCATION CONSULTANT • PALMETTO GBAElizabeth Brogdon has over 20 years of nursing experience. She is a graduate of Forsyth Technical College. Elizabeth holds a Bachelor of Science in Health Care Administration from Columbia Southern University. She has a background in Emergency Medicine in Nursing, Occupational Health and Home Health Care. Her educator experience includes Instructor for The American Heart Association and several other agencies. She previously worked for Palmetto GBA as a Medical Reviewer on the Pre-Claim Review Demonstration. Elizabeth is currently a Senior Provider Education Consultant with Provider Outreach and Education for the Review Choice Demonstration. Her duties include providing education for the Home Health and Inpatient Rehabilitation Review Choice Demonstrations through one on one and large group setting sessions.
Jessica Devine
DIRECTOR OF MEMBERSHIP GROWTH & RETENTION • HOME CARE ASSOCIATION OF FLORIDAJessica Devine has worked in home care since 2014 across various provider types, including Medicare-certified home health, nurse registry, and private duty. Prior to joining HCAF, she served as the Director of Private Care for a Visiting Nurse Association, overseeing a multi-region business in conjunction with the organization's home health and hospice services. She strongly believes that home care, more than any other form of health care, enhances patient well-being by promoting independence and preserving dignity, regardless of their living situation.
Jessica is a proud alumna of the University of Central Florida, where she earned a Bachelor of Arts in Interdisciplinary Studies.
Melinda A. Gaboury, COS-C
CO-FOUNDER & CHIEF EXECUTIVE OFFICER • HEALTHCARE PROVIDER SOLUTIONS, INC.Melinda A. Gaboury, COS-C, is the Chief Executive Officer of Healthcare Provider Solutions. With more than 30 years in home care, has over 22 years of executive speaking and educating experience, including extensive day-to-day interaction with home care and hospice professionals. She routinely conducts home care and hospice reimbursement workshops and speaks at state association meetings throughout the country. Melinda has extensive experience in Medicare Patient-Driven Groupings Model (PDGM) training, billing, collections, case-mix calculations, chart reviews, and due diligence. Medicare appeals (UPIC, RA, ADR, and TPE) with all the Medicare Administrative Contractors (MACs) have become the forefront of Melinda’s current impact on the industry. She is currently serving as the Chair of the National Association for Home Care & Hospice (NAHC) Home Care and Hospice Financial Managers Association (HHFMA) Advisory Board and Work Group, Treasurer of the Home Care Association of Florida Board of Directors and serves on the Tennessee Association for Home Care Board. Melinda is also the author of the Home Health OASIS Guide to OASIS-E and Home Health Billing Answers 2024.
Christina Crumbley, RN, MSN, HCS-D, HCS-O
FOUNDER & CHIEF EXECUTIVE OFFICER • TIDAL HEALTHCARE CONSULTING, INC.Christina Crumbley, RN, MSN, HCS-D, HCS-O, is Chief Executive Officer of Tidal Healthcare Consulting, LLC. Christina has 30 years of broad-based experience in home health operations, quality assurance/performance improvement, clinical management, finance, and business development, with 26 of those years spent in leadership positions. Through Tidal Healthcare Consulting, LLC, she provides consulting services that include operation and clinical management for home health services, ICD-10 coding, OASIS (Outcome and Assessment Information Set) review, and educational services. As a contract surveyor for the Accreditation Commission for Health Care (ACHC), she travels nationwide to conduct surveys for home health agencies and private duty agencies. She currently serves as the Region 7 Director on the HCAF Board of Directors.
Patti Heid, MSPT, COS-C
SENIOR DIRECTOR BEST PRACTICES & COMPLIANCE • HOME CARE ASSOCIATION OF FLORIDAPatti Heid, MPST, COS-C, is HCAF’s Senior Director of Best Practices & Compliance. She has been a Physical Therapist for over 30 years and has worked in the home care setting for 27 of those years. She believes that nothing can compare to the rapport that one develops with a patient while working with them and their families, in their own homes, helping overcome the “real-life” day-to-day challenges that they face. Over the years, Heid has gained valuable experience by fulfilling many key roles within the home care arena, from field clinician through the management level. All these roles have given her deep abiding respect and admiration for the good that home care providers can achieve.
Patti graduated from St. John’s University in New York in 1991 with a Bachelor of Science degree in Biology. She obtained her Master of Science in Physical Therapy degree from the University of Miami in 1993.
Calvin McDaniel
SENIOR DIRECTOR OF GOVERNMENT AFFAIRS • NATIONAL ASSOCIATION FOR HOME CARE & HOSPICECalvin McDaniel is the Senior Director of Government Affairs at the National Association for Home Care & Hospice (NAHC), where he leads advocacy efforts and maintains relationships with legislators and stakeholders. With a background in health policy, he previously served as a Health Policy Fellow in the U.S. House of Representatives. His expertise extends to health care technology, having worked as a Consultant at Cerner Corporation, advising on best practices and improvements in clinical workflows. Prior to that, he gained hands-on patient care experience as a CT and Radiologic Technologist. Throughout his career, Calvin has demonstrated a commitment to advancing health care delivery through policy, advocacy, and practical expertise.
Kyle Simon
SENIOR DIRECTOR OF POLICY, ADVOCACY & COMMUNICATIONS • HOME CARE ASSOCIATION OF FLORIDAKyle Simon is HCAF's Senior Director of Policy, Advocacy & Communications. His background encompasses nearly two decades of experience in health policy, advocacy, and politics. He has collaborated with federal and state policymakers on legislative and regulatory issues concerning Medicare, Medicaid, and prescription drugs. Moreover, he has provided support to members of Congress, managed a statewide political organization representing the President of the United States, and held leadership positions in various political campaigns. Additionally, he has served as an Adjunct Professor at Valencia College since 2012.
Kyle holds a Master of Science degree in Applied American Politics and Policy, as well as a Bachelor of Science degree in Political Science from Florida State University.
Kimberly Smoak, MSH, QIDP
DEPUTY SECRETARY & STATE SURVEY AGENCY DIRECTOR • FLORIDA AGENCY FOR HEALTH CARE ADMINISTRATIONKimberly R. Smoak, MSH, QIDP, has been with the Agency for Health Care Administration (AHCA) since August 1995. She is the Deputy Secretary of the Division of Health Quality Assurance. The Division licenses and/or certifies and regulates 40 different providers, including hospitals, nursing homes, assisted living facilities, and home health agencies. Ms. Smoak is also the State Survey Agency Director for the State of Florida. She holds a Master of Science in Health with an emphasis on Aging Studies and is also a Qualified Intellectual Disability Professional (QIDP). In 2009 and 2017, she received Survey & Certification Achievement Awards, presented by the Centers for Medicare & Medicaid Services (CMS) for her work in survey and certification.
Beau Sorenson
SENIOR CONSULTANT • VISIONBOUND INTERNATIONALBeau Sorenson has over 20 years of experience in health care. He has served as the Administrator of Advantage In-Home Care and in many capacities at First Choice Home Health & Hospice, including Director of Operations, Chief Financial Officer, Chief Operations Officer, and Chief Executive Officer. While at First Choice, he has extensively used the VisionBound system to lead change in his departments and throughout the company. As a result of these efforts, First Choice’s clinical outcomes went from the 33rd percentile to 99th percentile in two years while saving over $1 million annually in extra overhead costs.
Beau is an expert in software systems and has worked with Allscripts to beta test and develop major new features in their home care application, including integrations that allow for a fully electronic medical record. As a result of his efforts, First Choice led all home health and hospice agencies nationwide in adopting a fully electronic chart.
Beau has been actively involved in the home care and hospice community, serving as Legislative Director and President for the Utah Association for Home Care and serving on the Home Health Financial Managers Association Workgroup. Beau has also had extensive experience in politics and advocacy, serving as a Legislative District Chair, Vice Chair, and Education Chair in the Utah Republican Party. He has advocated for home health and hospice on Capitol Hill in Washington, D.C., and in Utah.
Beau is an expert presenter, and has given keynote and breakout addresses at conferences nationwide, in addition to teaching multi-day seminars as the sole presenter. His hands-on experience with the VisionBound system will provide participants with the real world knowledge and expertise they need to drive change throughout their companies.
Beau has lived and worked in Japan in addition to his home state of Utah which has given him a global perspective on business and health care. He currently lives in Provo with his wife, Rachelle, and their four children.
