Steps needed to schedule an appointment with an EOC Counselor/Advisor:

Is this your first time meeting with a EOC counselor?

1. Please complete the interest form below

2. EOC staff will review and contact you directly 

   1. If you are a NEW interested participant you will need to fill out an intake form that will be emailed to you via Adobe Sign. (due to regulations, a completed intake is required before a counselor can meet with you)

   2. After the required intake is completed, an EOC counselor will review and contact you to set-up  a virtual appointment
