The purpose of the caption sheet is to provide context for each assignment included in student's portfolios.
Caption Sheets must include all of the following:
Title of Assignment
Student Name
Common Core Standards. Write the standard that best fits the assignment. This should be a content standard for the individual subject area. A number such as 2.1 will NOT be accepted for the standard. Teachers can help identify the standards for any given assignment.
Schoolwide Learning Outcomes. Write out the SLO which best fits with the assignment.
Description of the assignment. Explain what the assignment required. Also, provide enough details so your readers can easily understand the assignment.
Reflection. Write 3 to 5 quality sentences reflecting on your work and your process. Some questions to consider include:
How does the assignment best meet the identified SLO?
What are you most proud of about the assignment?
What did you learn by completing this assignment?
How does this assignment connect to your future goals?
What is the significance of this assignment?
Where did you encounter difficulty with the assignment? How did you overcome the obstacles?
If you could do this assignment over, what would you do differently?
Students must include at least one meaningful assignment per course they have taken.
Students must create a copy of the following blank caption sheet for each assignment included in their portfolio.
