Updated ATD cards for those that have 12/31/25 expiration dates are needed if you have not submitted them yet.

As a reminder, if you have an upcoming visit with them this winter and have not submitted current flu shot documentation, you will be required to wear a mask during your visit. 

Everyone needs to sign and email the Visitor Confidentiality Form (either version is acceptable) for 2026. Form on right is the digital version.
