Dress Code for Upstate Therapy Dogs
· Polo shirt, solid color only, short or long sleeve, denim shirt, solid color turtleneck, or fleece may be worn with Upstate Therapy Dog monogrammed on the left breast. Contact Paula King for embroidery, cost is $12 per shirt. Put money and the shirt in a bag along with your name and contact info. Arrange with Paula how to get to her.

· Wear sensible, safe walking shoes, closed toe with backs or at least a strap around heel. NO flipflops, high heels, spike heels, sandals, or backless shoes.
· Due to allergies, do not wear perfume, cologne or scented lotion.
Facility Requirements
· Prisma Hospital all locations
o You MUST complete the orientation process to become a hospital volunteer prior to doing any visit at any Prisma location
o Contact Prisma Hospital Volunteer Coordinator, Deb Wyant at (864) 455-7994 or at deb.wyant@prismahealth.org to receive information on what this process involves and beginning this process.
o This process involves some initial paperwork, TB Test which Deb will set up for you, an online orientation course which takes a couple of hours and you can do from home on your computer. They do require the covid vaccine and annual flu shots. Once you completed the process, you will get a volunteer badge for you and your dog. These visits are very rewarding and we need volunteers! You can also get vouchers for lunch from Deb as well.
o After you have completed the Prisma Volunteer process, Deb will forward your name to Kim Greig, our Prisma Pet Therapy Visit Coordinator and she will contact you via email. Her email is etgreig@aol.com and Phone (864) 325-8051.
o Prisma Dress Code: Upstate Therapy Dog shirt, pants must be Khaki, tan,black, NO JEANS.
o Prisma Locations are: Greenville Memorial Hospital, Greer Memorial Hospital and Cottages, Hillcrest Hospital, North Greenville Hospital, Easley Baptist Hospital, Patewood Medical Campus, and Patewood hospital
· Shriners Hospital
o They require a copy of your ATD membership card and a yearly Flu shot. They also have a volunteer sheet that must be filled out. Contact Shriners Vounteer Coordinator Kaye Martell at kayemartell5@gmail.com for information about visiting.
o When doing a visit, you must have your drivers license which will act as your identification when visiting.
· Assisted Living Facilities
o They require a copy of you dog’s shot records, with proof of Rabies shot, to keep on file. Refer to the handout of Site visit coordinators and reach out to the person coordinating the visit at the facility you wish to visit.
