Update as of December 5, 2024: Below is a copy of the Background and Informed Consent form that was provided to the Tribal members and Tribal organizations that shared their input and knowledge for the Unmet Need Report.
When sharing input and knowledge, everyone is required to sign a Background and Informed Consent Form.
This form provides background information on the report and on the kinds of questions that might be asked during input opportunities, how input knowledge shared will used, protected, and shared back with you, how to edit or withdraw input/knowledge provided, and potential risks and benefits of providing input/knowledge.
