Dental, Vision and Variable claims can be attached and emailed to the following address with all relevant supporting documentation. Please DO NOT use this email address for questions regarding claims. This address is for claim submission ONLY.
Claims will be accepted in PDF format only. Pictures of claims will not be accepted. Click here for a video on how use the Notes feature on your phone to scan documents.
