Documentation will be requested by a Pre-K teacher once your application has been processed.  You will need the following documents:

• Child's birth certificate

• Child's social security card

• Proof of address (such as an electric/water bill)

• Proof of income (such as a check stub, W2, income tax return, or DHS letter with case number listed)

• TN Care card (if applicable)

• Custody papers (if applicable)

• Medical documentation for any health problems (such as allergies and impairments)- If your child receives an type of therapy, please bring documentation supporting this.

• Copy of Individual Education Plan (IEP-if applicable)
