Pre-departure information

You will need to login to "my study abroad" (https://app.studyabroad.illinois.edu/) and complete all required forms before departure. Some forms will be done online and some documents can be uploaded online or submitted in person to 2019 BIF.

The following forms in My Study Abroad are due by April 1 for fall programs and November 1 for spring programs:

• Signed Cost Acknowledgment Form (sign and either scan or take clear photo and upload to My Study Abroad OR turn in hard copy to 2019 BIF.)
• Pre-departure meeting will be marked complete after the meetings by the study abroad staff
• Read and electronically sign the six signature documents (submitted online)
• Complete the health and safety quiz (submitted online)
• Emergency contact (submitted online)
• Name, email and photograph release (submitted online)
• Visa questionnaire (submitted online)

Forms that may be submitted after the deadline after the April 1/November 1 deadline but must be submitted prior to departure to your host country:

• Photocopy of your visa (if visa is required). A scanned or legible photographed copy is acceptable (submit online)
• Travel itinerary (complete before leaving the country-submit online)

Course approval forms will not be due until the semester after your study abroad term.
