I. New Students (graduate of senior high)

• 3. Photocopy of NSO/PSA Live birth

  • (in the absence of the above requirements)

    • School I.D

    • 3rd Periodical Grades

    • Certificate of graduation

II. New Students fall under the category of grade 10 (high school graduate)

• 1. Form 138

• 2. Good Moral Character

• 3. Photocopy of NSO/PSA Live birth

III. ALS/PEPT

• 1. ALS/PEPT Certificate

• 2. Photocopy NSO/PSA Live Birth

IV. Transferee

• 1. Photocopy of Honorable dismissal / Proof of filing (claim slip) and TOR for evaluation

• 2. Good Moral Character

• 3. Photocopy of NSO/PSA

V. Cross Enrollee

• 1. Photocopy of Permit to Cross Enroll

VI. Returnee Student
