• TWL YoungLives Trip Leader Checklist (a helpful checklist of all the things!)

• Permission for Camper to Leave Early​ (if a camper needs to leave separately from the rest of the area, a Permission to Leave Early form must be completed by the camper's parent/guardian.

• TWL Medication Collection Form - please download and complete this form for every camper with meds.

• YL Connect Camp Arrival Form - Young Life is requiring the use of YL Connect Camp for camp registration, as well as the YLCC Arrival Form for health form tracking and housing.  YLCC allows you to manage registrations, payments, health forms, and housing on one platform.  Click HERE for YLCC Arrival Form instructions.

Mom and child both must be registered for camp and on the arrival form

YLVS Childcare should be utilizing their separate registration page

• Michigan Personnel Record Form: The State of Michigan requires this form for every leader.  Please arrive with this form filled out and ready to turn in at Welcome.  The good news is, if you've ever been a leader at TWL in the past and have filled this form out, we do not need another one!
