HEALTH & CONSENT FORMS

The new Health Form is here! ​ Please review the Health Form Page on Staff Resources for step-by-step instructions on inviting parents to complete the health form and reviewing submitted information.

*Paper Forms: paper forms are now campaign specific and can be found on your arrival form. Any old versions of the 6007 form will NOT be accepted at camp this summer. If a paper form is needed, be sure to use the correct and current version of your trip. 

Paper forms must be uploaded into YLCC. Please collect and upload any paper health forms prior to your arrival to camp. 

Spanish forms will be available the week of Feb 8, 2026
