1. หนังสือมอบอำนาจ และกรอกเอกสาร พร้อมลงนามให้เรียบร้อย

2. สำเนาบัตรประชาชน หรือ Passport เจ้าของเดิม ผู้มอบอำนาจ

3. บัตรประชาชนตัวจริง หรือ Passport ตัวจริง ของ เจ้าของเดิม ผู้มอบอำนาจ

4. บัตรประชาชนตัวจริง ของ ผู้รับมอบอำนาจ ที่มาติดต่อ

*โปรดทราบว่า อายุการใช้งานเบอร์ และ สิทธิ์การใช้งาน true points, dtac coin จะสิ้นสุดทันที และจะเริ่มนับ 1 ใหม่ กับลูกค้าท่านใหม่ ครับ

- Original ID card or Passport of the original owner (the authorizer)

- Original ID card of the authorized representative who comes to the shop
