Re: Account of John M. Doe; Account Number 12345678910
Dear Sir or Madam:
I am writing to report that you did not properly credit my account with the payment I sent you last month (June, 2002). I sent you a payment of $160.00, but my statement indicates that I made a payment of just $60.00.
I have attached the following documents as proof of your mistake:
(1) A copy of the front and back of my cancelled check in the amount of $160.00;
(2) A copy of my June statement with the incorrect $60.00 payment notation circled in red.
Please investigate this matter and correct the billing error as soon as possible. Thank you.
Sincerely,
John Doe

Re: Account of John M. Doe; Account Number 12345678910
Dear Sir or Madam:
I have just received my August 2002 statement and noticed that the billing error I reported to you last month has still not been corrected. I sent you a certified letter on July 16, 2002 informing you that you did not properly credit my account. A copy of that letter and my certified mail receipt proving you received my first letter are attached to this letter.
Once again, I an informing you that I sent you a payment of $160.00 in June, 2002, yet, you credited my account for only $60.00. I have once again attached copies of the following documents as proof:
(1) A copy of the front and back of my cancelled check in the amount of $160.00;
(2) A copy of my June statement with the incorrect payment amount circled in red.
Please comply with the Fair Credit Billing Act and correct your mistake as soon as possible. I have better things to do with my time than send you certified letters. If this error is not corrected on my next statement, I am going to report the matter to the proper regulatory agency.
Sincerely,
John Doe

Re: Account of Jane Doe; Account Number 0000000000
Dear Sir or Madam:
I just received my July statement and was surprised to discover that you still have not reversed a charge that does not belong to me and which I reported to you last month. I have attached a copy of my first letter for your review.
As I stated in my first letter, this item appears on my June statement and is referenced no. X575758595959. It was paid to Acme Industries in the amount of $470.00. I did not make this charge and have no idea why it has been attributed to my account. I am once again requesting that this matter be investigated and the error corrected as soon as possible.
In accordance with the Fair Credit Billing Act, you are required to correct such errors within 60 days of them having been reported to you. If this error is not corrected within that time period, I will have no choice but to report this matter to the proper authorities.
Sincerely,
Jane Doe

Re: Account of [your name]; Account Number ___________
Dear Sir or Madam:
I am writing to request that the $29.00 late fee placed on my account last month be reversed. I did not send in my payment late. My check register shows that I mailed my payment on [date], which is at least two weeks before the due date. If you did indeed receive it late, it must have been the fault of the post office.
My payment history with your company shows that I have never sent in a late payment before. I value my credit rating and always pay my bills on time. Therefore, I am requesting that this late payment be reversed and that you notify me in writing that this has been done. Thank you for your attention to this matter.
Sincerely,

I understand that I have the right to continue to use my credit card under the existing terms until the expiration date on my credit card at which time my account will be closed. I also understand that I may pay off any remaining balance under my current terms.
