Eclipse Forms

Download the app from the link below depending on if you have an Apple (IPhone, IPad) or an Android Phone (Samsung, Motorola, Google, LG). Hit allow on all permissions during installation. You will only need to follow steps 1-3 once.

1. Download and open the app above

2. Copy the service URI from below and paste it into the "Service URI" box (You can copy it by holding down on the link and pressing "Copy":

3. Register the device by entering your username, password and device type. Note that username and password are case sensitive and all usernames are all capital letters

4. Now that the device is registered, enter your login credentials one more time

5. Select "Others" and you should see all forms your login has mobile access to
