This survey was conducted as part of an assignment for my ISM Class. The idea of this survey was to get the public's knowledge and opinion on the accessibility of speech services. As I received responses, the survey was requested in Spanish for parents who spoke primarily Spanish.
A Speech Language Pathologist wants the evaluations to be as easy to understand but there are still terms that are used. Attached is a graphic made by myself with some basic definitions of some commonly used.
A Case History form is a form used to evaluate a student before their formal evaluation. These are used to contact the parents, teachers and other adults that a child interacts with to provide their opinion about the student's speech needs. The form below is used in pediatrics or for kids and is available in English and Spanish. This form was created by Super Duper Speech Publications.
