Welcome to the forms tab! This is where all sign up, scheduling, registration, and all other forms will be to fill out.
Parent Forms
This sign up form allows us to know the general information required for us to have your child(ren) within our care as well as daily schedule for which you believe they may go by. If their schedule ever changes feel free to let us know. We keep track of daily attendance for invoicing purposes and it is not a concern if it changes at all!
You must fill out one form per child.
This doesn't not sign your child(ren) up for summer rec but does let us know what they are involved in so we can ensure they get to their events.
