NBI Clearance

Original and photocopy of the following:

• Filled-out Application form

• Valid Passport

• 1 pc. 2x2 size picture (front view)

• NSO/PSA Marriage Certificate for married Filipina who has changed her surname

Note : The NBI form is not a consular document. However, the Philippine Embassy assists Filipinos who are applying for NBI clearance in terms of fingerprinting on the card form. Upon completion of the requirements, the applicant must send the form, together with an authorization letter or Special Power of Attorney to his/her authorized representative in the Philippines, who will process the clearance with the NBI on your behalf.
