If Filipino parent/s acquired citizenship and re-acquired citizenship, submit certificate of naturalization of parents and reacquisition certificates of parents and of the child. If the child is 18 years old and above at the time of the reacquisition of Philippine citizenship, submit an Identification Certificate for Filipino Citizen issued by the Philippine Bureau of Immigration.
Delayed Report (more than 12 months after the child has been born)
ALL above requirements plus Notarized Affidavit of Delayed Registration
Negative Report of Birth (original) on security paper issued by the Philippine Statistics Authority in Manila (valid for one year upon registration)

a. If the child is six (6) years old or below, the mother or, in her absence, the legal guardian, shall execute the AUSF.
b. If the child is seven (7) years old or above up to seventeen (17) years old, the child shall execute the AUSF with an accompanying attestation of the mother or the legal guardian.
c. If the person is eighteen years (18) old or above, the person shall execute the AUSF without an attestation of the mother or legal guardian.
Father to execute a Notarized Affidavit of Acknowledgement of Paternity (APP) in four (4) originals

Notarized Affidavit of Parental Consent executed by both parents of Filipino applicant who is 18 to 20 years old. If parents are not residing in Romania, the notarized Affidavit of Parental Consent shall be authenticated by the Department of Foreign Affairs (DFA) or the Philippine Embassy or Consulate nearest the place of residence of the parents.
Notarized Affidavit of Parental Advice executed by both parents of Filipino applicant who is 21 to 25 years old. If parents are not residing in Romania, the notarized Affidavit of Parental Advice shall be authenticated by the Department of Foreign Affairs (DFA) or the Philippine Embassy or Consulate nearest the place of residence of the parents.
NOTE:
The CLCCM/ Marriage License will be released only after the marriage banns have been posted on the Philippine Embassy bulletin for ten (10) consecutive days.
Contracting parties are advised to file as soon as possible a Report of Marriage with the Embassy
