Civil Registry

Original and five (5) photocopies of the following:

• Personal appearance of one of the parents

• Filled-out Application form

• Foreign Birth Certificate with English translation

• NSO/PSA issued Marriage Certificate/Report of Marriage

• Passport of Filipino parent/s (valid at the time of birth of the child) – Photocopy of passport data page. In case of unavailability, execute an Affidavit of Non-Submission of document and attach a copy of any valid or recent passport.

• Passport of Filipino parents (current/valid passport) – photocopy of data page. In case of unavailability, execute an Affidavit of Non-Submission of document and attach a copy of any valid ID.

• Five (5) recent passport-size photos of the child

• If Filipino parent/s acquired citizenship and re-acquired citizenship, submit certificate of naturalization of parents and reacquisition certificates of parents and of the child. If the child is 18 years old and above at the time of the reacquisition of Philippine citizenship, submit an Identification Certificate for Filipino Citizen issued by the Philippine Bureau of Immigration.

Delayed Report (more than 12 months after the child has been born)

• ALL above requirements plus Notarized Affidavit of Delayed Registration 

• Negative Report of Birth (original) on security paper issued by the Philippine Statistics Authority in Manila (valid for one year upon registration)

Original and five (5) photocopies of the following:

• Personal appearance of the child and both of the parents

• Filled-out Application form

• Foreign Birth Certificate with English translation

• Passport of the Filipino parent/s (current/valid) – photocopy of passport data page. In case of unavailability, execute an Affidavit of Non-Submission of document and attach a copy of any valid ID. 

• Passport of Filipino parents (current/valid passport) – photocopy of data page. In case of unavailability, execute an Affidavit of Non-Submission of document and attach a copy of any valid ID.

• Five (5) recent passport-size photos of the child

PLUS

• Mother to execute a Notarized Affidavit to Use the Surname of the Father (AUSF)  in four (4) originals

  a. If the child is six (6) years old or below, the mother or, in her absence, the legal guardian, shall execute the AUSF.
  b. If the child is seven (7) years old or above up to seventeen (17) years old, the child shall execute the AUSF with an accompanying attestation of the           mother or the legal guardian.

        c. If the person is eighteen years (18) old or above, the person shall execute the AUSF without an attestation of the mother or legal guardian. 

• Father to execute a Notarized Affidavit of Acknowledgement of Paternity (APP) in four (4) originals 

• If Filipino parent/s acquired citizenship and re-acquired citizenship, submit certificate of naturalization of parents and reacquisition certificates of parents and of the child. If the child is 18 years old and above at the time of the reacquisition of Philippine citizenship, submit an Identification Certificate for Filipino Citizen issued by the Philippine Bureau of Immigration.

If the child is 18 years old and above at the time of the reacquisition of Philippine citizenship, submit an Identification Certificate for Filipino Citizen issued by the Philippine Bureau of Immigration.

• DFA Apostilled Death Certificate and previous PSA Marriage Certificate if the parent is a widow/er

Original and four (4) photocopies of the following:

• Personal appearance of both contracting parties

• Filled-out Application form

• Foreign Marriage Certificate with English translation

• NSO/PSA Birth certificate (for Filipino)

• Birth Certificate issued by the country of birth (for foreign national) with official English translation

• Passport of both parties valid at the time of marriage – photocopies of data page

• Passport of both parties (current/valid). In case of unavailability, execute an Affidavit of Non-Submission of document and attach a copy of any valid ID.

• Five (5) recent passport-size photos

Delayed Report (more than 12 months after the marriage)

• ALL above requirements plus Notarized Affidavit of Delayed Registration 

• Latest Certificate of No Record of Marriage issued by the Philippine Statistics Authority (PSA)

NOTE:

• For annulled Filipino spouse, submit a copy of DFA Apostilled Marriage Contract issued by the PSA with proper Annotation

• For divorced Filipino spouse, submit a copy of the Apostilled Divorce Decree/Certificate with judicial recognition from a competent Philippine Court

• For widowed Filipino spouse, submit an Apostilled Death Certificate issued by the PSA or foreign-issued death certificate of previous spouse (with English translation, if applicable)

• For divorced, annulled, or legally separated foreign spouse, submit a copy of foreign-issued decree/decision (with English translation, if applicable)

• For widowed foreign spouse, submit Death Certificate of previous spouse (with English translation, if applicable)

Original and photocopy of the following:

• Filled-out Application form

• DFA Apostilled NSO/PSA Birth Certificate

• DFA Apostilled NSO/PSA Certificate of No Marriage (CENOMAR)

• DFA Apostilled NSO/PSA Marriage Certificate and Advisory on Marriages with Annotations for previously married (annulled or divorced) applicants (including former Filipinos or dual citizens at the time of divorce)

• Valid Passport of both parties

• 1pc. Passport-size picture of Filipino parties

• Notarized Affidavit of Parental Consent executed by both parents of Filipino applicant who is 18 to 20 years old. If parents are not residing in Romania, the notarized Affidavit of Parental Consent shall be authenticated by the Department of Foreign Affairs (DFA) or the Philippine Embassy or Consulate nearest the place of residence of the parents.

• Notarized Affidavit of Parental Advice executed by both parents of Filipino applicant who is 21 to 25 years old.  If parents are not residing in Romania, the notarized Affidavit of Parental Advice shall be authenticated by the Department of Foreign Affairs (DFA) or the Philippine Embassy or Consulate nearest the place of residence of the parents.

NOTE:

• The CLCCM/ Marriage License will be released only after the marriage banns have been posted on the Philippine Embassy bulletin for ten (10) consecutive days.

• Contracting parties are advised to file as soon as possible a Report of Marriage with the Embassy

Original and four (4) photocopies of the following:

• Filled-out Application form

• Death Certificate (with English translation, if applicable)

• Certificate of Sealing of Casket

• Autopsy/Embalming Report

• Latest Passport of the deceased

NOTE:

Personal appearance of person authorized to report the death is required

Original and four (4) photocopies of the following:

• Death Certificate (with English translation, if applicable)

• Embalming Certificate or Certificate of Cremation (with English translation, if applicable)

• Certificate of Non-Contagious Disease (with English translation, notarized and Apostilled by Romanian competent authorities) 

• Special Power of Attorney executed by the next-of-kin (spouse, parent, or nearest family member, in that order) and transmitted officially to the Embassy, through the DFA, authorizing an Attorney-in-fact to coordinate in the shipment of human remains or cremated remains

• Letter of Acceptance of Human Remains from the next of kin (spouse, parent, or nearest family member, in that order), transmitted officially to the Embassy through the DFA

• Passport or ID of the Representative

• Flight Itinerary
