Are you able to share your time, expertise, or skills with the PTA?
All "Essential Volunteers" on campus need the following turned in to Nancy in the office:
Fingerprints on file with the district. You need to complete the form below and bring it to the Capitol Live Scan office on Broadway to get fingerprints. The cost is $67. Click here to request financial assistance
A recent negative TB test (within the last 5 years)
A copy of your COVID-19 vaccination card
The Volunteer packet (a new one must be turned in each year). See below to click, print, fill out, and return to Nancy
Complete Mandated Reporter Training (click here) and email the certificate of completion to Nancy
