1. Visit the Parish Office on Saturday or Sunday after Mass, fill and submit the registration form

2. OR, Download the form above, fill and submit to the Parish Office 

After submission of form, you will receive a Parish ID on your email, collect the Church Support card from the parish office for tracking your donations and support
